If you have questions regarding the Chapel of Perpetual Adoration, or would like information about committing to a weekly hour, about becoming a coordinator, or about being an occasional substitute, fill out the form below.  Someone from the Chapel will contact you within 24-48 hours.

Please DO NOT use this form to notify your coordinator that you are unable to be at your scheduled hour and have been unable to find a substitute.  That should be done in a phone call, as soon as possible.  Thanks!
